Our Story

Life Health and Wellness Health Care Medical Clinic began in Phoenix in 2003 to serve the health care needs of the Community. Our Veteran contracts allow us to serve Veterans, Active-duty Military, Reservist and their families. We also serve our community, any persons in need of health care. Our current location is 5040 N 15 ave, Suite 105, Phoenix AZ. This location has experienced health care providers. Life Health and Wellness Health Care providers. All services are provided at each facility. Our facility provides the best care possible while maintaining HIPAA (Health Insurance Portability and Accessibility Act) compliance for our patient’s electronic health records.

Our Doctor

Dr. Beverly Thomas-Carter, Our Top Doctor of America, (see press release) has been caring for patients in the medical field for 32 years. Doctorate Nurse Practitioner (DNP) graduate from Arizona State University, Dr. Beverly practices family medicine as an independent practitioner. She has a specialized

interest in adolescent mental health and her specialty is providing medical care for the entire family (newborns to the elderly).

Dr. Beverly has also completed advanced practice medical fellowships in pain management and pediatric pulmonary diseases. Dr. Beverly is also a part-time professor teaching students to complete their degrees in the medical profession at many area universities.

Our Team

Our integrative team consist of a medical

team and a behavioral health team of

professionals. The medical team consists of nurse practitioners with years of experience treating medical patients using evidence-based treatment plans and We provide referrals and collaborate with behavioral health teams consists of psychologists and behavioral health technicians trained in traumatized patient management and treatment and many other mental health issues. We offer mental health referrals for evaluations and treatment. On our home page you will find the Crisis Hotline in case of Emergencies.
